Rose Breslin Associates, LLC (the “Owner”) is now accepting Applications, on a first-come first-served basis, for 1-bedroom affordable rental units (the “Affordable Units”) in Yaphank, New York within the rental complex known as “The Reserve at the Boulevard.”
Pursuant to Town of Brookhaven requirements, the Affordable Units will be available to households with incomes that do not exceed 80% of the HUD Nassau/Suffolk area median income as set forth below based upon unit type.
APPLICATIONS FOR RESIDENCY MUST BE SUBMITTED DIRECTLY TO OWNER AND NOT TO LIHP

NOTE: Owner requires the following documentation to be submitted with the Application:
1) Signed Qualification Acknowledgement
2) Proof of Income-please contact leasing office for details by calling 631-448-7770
3) $20 Application fee per applicant over the age of 18.The Long Island Housing Partnership, Inc. (“LIHP”) is retained by Owner in a limited capacity to review applicant maximum income eligibility.
APPLICANTS ARE ADVISED THAT IF APPROVED BY OWNER, APPLICANTS MUST THEN BE REVIEWED FOR INCOME ELIGIBILITY BY LIHP.

LIHP IS NEITHER THE OWNER, LANDLORD, MANAGER OR LEASING AGENT. FINAL ELIGIBILITY FOR TENANCY AND OCCUPANCY IS THE SOLE DETERMINATION OF THE OWNER. LIHP’S ROLE IS LIMITED TO INCOME QUALIFYING APPLICANTS. ANY REQUEST FOR A REASONABLE ACCOMMODATION REGARDING ANY UNIT MUST BE CONVEYED TO AND PROCESSED BY THE OWNER.
